Help making rings?

Today I was gonna make a proto-planetary disk but there were 2 problems, first why is my ring always tilted? Second the disk was at 0.0800 distance at inner and I had a ring over 5 distance away from my star (Sun)  :o
I also have wondered whether I can make rings out of crashing things into other planets..Usually when I crash things into planets (Example, the Moon formed when a Mars sized object hit a larger Earth, mars just sinks into Earth when I try to remake it ???). Anyone have an idea how I would make a ring out of collisions and with the problems I had?

I've tried as well.

The closest I've come to succeeding is exploding asteroids near a planet, though eventually all the debris and fragments go elsewhere. 

I'm thinking there are just too many data points to track.
