The appearance of a wormhole would probably be like a crystal ball; a sphere that you could see the end location through.

Besides, there's only a one way entrance and exit with wormholes(Because wormholes are made by connecting a black hole & a white hole.) So White holes must be added to create a wormhole.

It's true (Hypothetically), but it's not the only way. Inserting exotic matter into the Planck-Scale Quantum Foam loops should enlarge them enough to be wormholes, or they could just be floating around in space just because.
