Title: Game not exiting sometimes after sending feedback or survey

Seems to be intermittent problems with this exit/feedback thing in 17.1 and 18c8....  Sometimes it works, sometimes it doesn't come up at all.  Other times, I am prompted for feedback, but no user input is accepted.  And, other times, user input IS accepted, but clicking OK or SUBMIT just causes the screen to begin to fade away, then freeze and UBox 2 never exits, and must be forcibly killed, which I hate doing because it means the simulator was in an unknown state when closed and that could damage it.

Perhaps we could just dispense with this needless annoyance and add a "feedback" button to the main control panel that we can use when we have something we need to report feedback on?

I've seen this happen, but am not sure what's happening. Force quitting isn't going to damage the simulation or save states.

Has anyone else seen this problem?
